People experiencing mild to moderate mental health challenges, harm from alcohol, drugs or other behaviours can find free and fast support at the following GP clinics (see below) from Wellbeing Teams. The team is made up of Health Improvement Practitioners (HIPs), Health Coaches, Awhi Ora Community Support Workers and your GP and nurse. Call or email your GP clinic to make an appointment. 

Health Improvement Practitioners (HIPs)
HIPs are specially trained registered practitioners, such as psychologists, nurses, occupational therapists and social workers. Their job is to assess your needs, develop a plan with you, provide evidence-based health interventions and coordinate support activities.

Health Coaches
Health Coaches come with diverse backgrounds and cultures and often have 'lived experience' of learning to live well with long-term health issues. They are trained to help you understand your health issues and learn to manage them better. They will support lifestyle change, provide emotional support, help you access services and act as your advocate.

Awhi ora Community Support Workers
Where the Health Improvement Practitioners and Health Coaches are based primarily in clinic, Awhi Ora is the link to all the support and services in the community.

Awhi Ora is a flexible and person-centred wellbeing support service, which supports people to identify and work on the challenges that are important to them. The service is free and provided by a collaborative of mental health NGOs, which specialise in providing community, cultural and peer support for people with mental health and wellbeing needs. Awhi Ora takes a preventative, early intervention approach, broadening access to people experiencing distress or life challenges, as well as those with a diagnosed mental health or addictions illness, but who are not under secondary care.

GP and practice team
Your GP and nurse are a key part of the wellbeing team as they understand your health needs and know about your health history. The GP and practice team work with the wellbeing team and provide advice and guidance for your care. They have direct links to DHB specialist services, mental health and addictions services and other health services, if required.
